Massive Baclofen overdose

Dear Ask The Doctor: What is the antidote or treatment for an overdose disgestion of bachlofen 25mg (dose about 1 grams)?
the patient is in a coma but she seems to be responding to our voice by crying, increased heartbeats ,and contraction of the muscles of the eyebrows
Dear Ahmad: The maximum oral baclofen dose is 80mg/day; therefore administration of 1gram constitutes a massive overdose. Please attend the nearest emergency room immediately. Treatment of a baclofen overdose is supportive with respiratory support if necessary, in an intensive care environment. Anecdotal reports suggest that i.v. physostigmine may reverse central side effects, notably drowsiness and respiratory depression. However caution is advised in administering intravenous physostigmine, because its use has been associated with the induction of seizures, slow heart rate, and heart rhythm disturbances. There are also some case reports of atropine use for slow heart rate and low blood pressure, due to baclofen overdose. Please seek urgent medical attention or call 911. |
