Dear Mohammed: I do understand your concerns for problems with dysfunction. The three major forms of male sexual dysfunction : erectile dysfunction, ejaculatory dysfunction and decreased libido. The first step in the treatment of male sexual dysfunction is to determine the etiology. Certain clues in the history and appropriate laboratory testing can usually establish the primary cause.

The most common cause of premature ejaculation (PE) is psychogenic. Increased anxiety, stress, depression may all cause PE. Certain vascular and neurogenic diseases like diabetes and hypertension may also be a contributing factor. It is important to find the cause of PE before starting any therapy. Decreasing stress and anxiety and more talking to your partner regarding your concerns and fears will help. If these measures do not help then pharmacotherapy is the next best option. Psychotherapy alone or in combination with psychoactive drugs may be helpful in men whose erectile dysfunction is caused by depression or anxiety. In addition, alpha adrenergic blocking agents such as yohimbine hydrochloride may be used as a remedy for men with psychogenic erectile dysfunction.
