HSV Type 1 and Type 2: Effect of Creative Sexual Practices on its Transmission

Dear Ask The Doctor: If I kissed a guy who has oral HSV2 am I at risk for genital herpes? I have no oral symptoms but did find 2 small pimple like bumps on my genitals.

Herpes is caused by Herpes Simplex virus type 1 and type 2. Long before, HSV type 1 more commonly causes infections on the mouth (lips, pharynx, gums) while HSV type 2 usually causes genital herpes. However, in modern times, HSV type 1 and 2 can be harboured in either areas of the body due to creative sexual practices (e.g. fellatio and cunnilingus). This viral infection typically recurs on the same area since it stays on the nerve. What I am trying to say is if he kissed you, you may contract HSV 2 and will have cold sores on your lips. If he performed cunnilingus on you, there is a chance you will get genital herpes. I would not know if those pimple-like bumps you mentioned are HSV lesions (they are usually ulcers w/ clear discharges) or folliculitis (inflammed hair follicle). The incubation period (time interval between exposure to disease and manifestation of signs and symptoms) of HSV infection is usually 3-7 days. If it really bothers you, have yourself checked by a doctor so that someone with clinical experience can actually assess the pimple-like bumps, then ask about the options for diagnosing and treating herpes.
