Phlebotomy is the Mainstay of Treatment in Polycythemia Vera

Dear Ask The Doctor: I am a 64 yr old male in a monogamous relationship who has polycythemia vera. Normal treatment is to donate blood periodically, however blood banks will not allow me to donate because I am gay. I do not feel that lying about my lifestyle is the way to go. Are there any other treatments or suggestions. My family doctor was of little help.
Dear James: Polycythemia vera is a blood disorder in which the bone marrow makes too many red blood cells. It can also be associated with high levels of white blood cells and platelets. Treatment focuses on treating symptoms and reducing thrombotic complications by reducing red blood cell levels. The removal of blood from the body (phlebotomy) reduces the blood volume and brings down the hematocrit levels; and in addition to low dose aspirin this reduces the risk of blood clots. I understand it is difficult in your particular case to donate blood. However your family doctor could arrange regular phlebotomy or referral to a haemotologist to help reduce the risk of future complications. |
