Breast Lump in Pubertal Males

Dear Ask The Doctor: Hi I am a 14 year old boy, i have a lump behind my right nipple that i noticed about a week ago, since this time the lump seems to have grown. It was sore to the touch but not anymore, I am not over weight by any means, I am around 98 pounds, im very active i row and play ball hockey i am very worried about this lump and hope that you can help relive the stress of all this, its at the point where im not sleeping at night and i feel sick to my stomach. First of all, let me have a brief comparison between the female and male breasts. The female breasts have ducts and lobules; the males only have ducts. Since you are at the stage of puberty, your testosterone level is relatively high. It is a complex biochemical process but, in normal states, even in males, testosterone can be converted to estrogen; and estrogen has a positive effect (enlarge) breast ducts (which is present in both males and females). This could be the possible cause of the mass behind your right nipple (high testosterone => converts more estrogen => possible enlarged breast ducts behind nipple). Another benign possibility is trauma. Since you also play contact sports like hockey, your right breast (or nipple) might have been hit, it was inflamed (sore) and now, probably, scar tissue (lump) has formed. If it is really bothering you, have it checked by a doctor because it is only by then you can be sure it is benign and there is no serious underlying condition. I do hope this helps.
