Chronic Anal fissure - What management should I take?

Dear Ask The Doctor: Hi, I have been suffering from an anal fissure for 6 years, the birth of my first child seemed to cause this! I have had a botox injection 2 years ago but this hasn't seemed to work. I have since seen another consultant who said put rectogesic cream on the area and to increase my fibre intake and fluid both of which I have done. I have been clear for 6 months approx but this week the problem has reoccurred. What I wanted to know is, when the fissure appears, I take fybogel sachets for several weeks until the fissure has healed. Would movicol be better than fybogel as I have read on the internet that fybogel can cause constiption if fluid isn't increased? I only take medication when the problem occurs. This problem is really getting me down and I can't understand why it keeps flaring up. Many thanks Jenete
Dear Jenette: I do understand your concern. It can be quite distressing at times when medical management options provide little relief. Usually the goal of management in the treatment of anal fissures is to relieve the constipation by using stool bulking agents (fybogel) and laxatives (movicol). I would advise you to continue with fybogel and supplement it with movicol as needed. Sitz bath after bowel movement may also provide symptomatic relief.
You may also consult your doctor whether a surgical option (lateral internal sphincterotomy) would be a good option in the treatment of your chronic anal fissure. Studies have shown the lateral internal sphincterotomy is more effective than botox injections in healing chronic anal fissures.
