What is the risk of general anesthesia on a patient with COPD and asthma?

Dear Ask The Doctor: Hi, I have moderate COPD with asthma and I need a gall-bladder operation. What are the risks of having a general anaesthetic?

Dear Paulette: Every operation has its risk and benefits. We need to weigh these option and see what is best for us. In regard to your question, the complication of general anesthesia on a patient suffering from COPD and asthma may cause an increase risk of post-operative pulmonary complication due to suppression of the respiratory drive by the anesthetic agent. COPD is related with an increased risk in upper airway obstruction and difficult extubation. Patient with COPD and asthma warrant for aggressive pre-operative management, including use of bronchodilators and glucocorticoid agents, and cessation of smoking.
