Diagnosis of Irritable bowel syndrome

Dear Ask The Doctor: What is wrong with my stomach? Okay, i have had "stomach" problems since i was in 6th grade, im now 21 and nothing has improved. When it first started, i went to a couple different doctors and they didnt diagnose anything, i had to take the test to see if i was lactose intolorant, and it was negative. i would just like to know what is wrong. I go back and forth between being constipated for days to having diarrhea every day. it doesnt matter what i eat, although greasy, spicey, and dairy foods tend to make things worse. Does any one have any ideas?? i was told IBS, but im not satisfied with that.
Dear Amy: The presence of irritable bowel syndrome can only be diagnosed in the absence of other organic disease and requires a detailed history and clinical examination by your family doctor, with reference to specific guidelines. It may be helpful to note in a diary your symptoms to establish a relationship, including duration of diarrhea or constipation, food ingested, the presence of any blood or mucous associated with the stool, associated symptoms (nausea or vomiting). Your symptoms could be due to a food allergy, malabsorption syndrome, inflammatory bowel disease or irritable bowel disease. I advise attending your family doctor for a detailed history and clinical examination, and following this he may be able to exclude many of the above diseases as a cause for your symptoms. |
