Dear Ask The Doctor: I have had RUQ pain, nausea, bloating or distention,and fatigue for the past 3 weeks. I have gone to the ER, GI, and PCP. They have given me 2 CTS, ultrasound, numerous blood works, and an abdominal xray. The xray showed that i had constipation but did a flush 2 1.2 weeks ago and fixed that. I still have all the symptoms though. All the tests came back negative except for my liver enzymes that were over 500 on the first test and down to over 400 on the next test. Also my amylase was on the high side of normal and my lipase was a little high at 68. The ER and GI said it was probably just IBS. I don't believe this because I have not had constipation, since the flush, and have had no diarrhea in years. My PCP says there is definitely something wrong but they are only able to do so much. My PCP wants me to go to the federally funded hospital, Ben Taub I live in Houston. She said that I have an abnormality and they would love my case. I am getting depressed because I am in constant pain, more when I eat. I have been on a liquid diet for a few days and am still having some pain and nausea. I don't have any insurance and can't afford to go see any non-funded doctors. Please help clue me in on what might be going on. Dear Shannon: I agree with your PCP that you should have a follow up for the increase liver enzymes and the pain in your RUQ. Liver enzymes increase when the liver is inflamed, this could happen by different causes such as viral infections, obstruction of the billiary ducts or as a side effect of medications that you might have taken. Medications such as ibuprofen have been associated with elevations in liver function tests in up to 15% of patients. Rarely, jaundice, cholestasis, hepatitis, and hepatic failure have been reported. I would suggest you to consult your physician about holding your medications associated with liver damage and test you again after a few weeks to rule out this possibility, in the meantime avoid alcohol, ibuprofen and fatty foods.
