Cardiac enzyme (Troponin I) elevation duration

Dear Ask The Doctor: I am a 37 year-old male. I am 5'10" and around 150 pounds. I dont have any health problems except for acid reflux. On Sunday 4-10-2010 i woke up at 2 am with a chest pain in the middle of my chest, it felt tight too. The next day on Sunday 4-11-2010 I noticed mey chest felt sore. Monday 4-12-2010 it was still sore. At about 6 pm on Monday i went to the ER. The ER doctor ran chest x-rays on my chest, a EKG, blood test, and had me hooked up to a heart monitor for three hours. The doctor came back in and told me the test were all good. And said I was good to go home. The blood test was performed 67 hours after my first chest pain that was on Saturday 4-10-2010 at 2 am, would the blood test still be reliable since 67 hours passed since the first initial pain? Would the troponin enzyme go back to normal level at 3 or 4 days if it was elevated? Thank you so very much for your help.
Dear Jason: Troponin I is reliably elevated 12 hours after heart muscle damage and remains elevated for a further 5 to 9 days. Therefore if this test was negative it is reassuring. Of course troponin only helps diagnose heart muscle damage and does not completely rule out heart artery disease which would require further investigations such as exercise stress test, perfusion scans or coronary angiography. |
