Dear CLARA: Pain in the lower back can be produced by vertebrae problems in the lumbar spine, discs between the vertebrae, ligaments around the spine and discs, spinal cord and nerves, muscles of the low back, and internal organs of the pelvis and abdomen. Therefore,the treatment of lower back pain is directed to, first of all, diagnosing ther suspected or specific cause. For acute lumbar strain, the initial approach is conservative , meaning: resting the back (to avoid reinjury), medications to relieve pain and muscle spasm, local heat applications, massage and eventual (after the acute episode resolves) reconditioning exercises to strengthen the lower back and abdominal muscles. Initial treatment at home might include heat application, anti inflammatory medication(Advil, Motrin), and avoiding reinjury and heavy lifting. Long periods of inactivity in bed are no longer recommended, as this treatment may actually slow recovery. Spinal manipulation for periods of up to one month has been found to be helpful in some patients who do not have signs of nerve irritation. Future injury is avoided by using back-protection techniques during activities and support devices as needed at home or work. If after a period of time that you follow these recommendations, you still have the lower back pain, a complete evaluation is strongly recommended to rule out other possible or more serious causes of the pain and accordingly proceed with the specific treatment. |
