Target blood pressures in the general population and specific diseases

Dear Ask The Doctor: My husband has been taking lisinopril for approximately 5 weeks to treat mild hypertension (140's/90's). His blood pressure readings have not changed during that time. His doctor ran a metabolic profile which was normal. Any thoughts?
Dear Angie: The current target BP is <140/90mmHg for the general population. A number of clinical trials and guidelines suggest a goal blood pressure of <130/80 mmHg in three clinical settings: diabetes mellitus; proteinuric chronic kidney disease; and atherosclerotic cardiovascular disease. If you have undertaken lifestyle modifications in addition to Lisinopril for a trial 3 month period, and your BP remains high, I would recommend attending your family practitioner regarding control with an alternative medication or addition of another agent such as a thiazide diuretic or calcium channel blocker. |
