Bladder Infection after five days treatment, back to ER

Dear Ask The Doctor: I am a forty seven yo female. I went to the ER five days ago for pain in my back, legs and stomach. I couldn't sleep, eat had diarreah and a severe headache. The Dr told me I had a bladder infection. I am still very sick, I can't eat without diarreah, my body still aches, I can't sleep for more than an hour or two. Should I go back to the ER, I don't think I should still be this sick with the anitbiotics..Could there be something else wrong with the above symptoms?
