Are these symptoms something I should see a GP about?

Patient : For the last month I have been going to the bathroom a lot more than usual, in both ways. I've been slightly bloated and I think my face is slightly puffy. After weighing myself it looks like I have lost 5lbs despite eating a lot more than usual for the last few months and exercising much less. I did a urine dip test and it looks like I have a moderately high amount of white blood cells and protein in my urine as well as an increased specific gravity. What could be wrong and is this something I should be concerned about?
