Chronic lower back pain

Patient : I have had chronic low back pain for years and is getting to the point where I can no longer stand the pain. I had an MRI done about a week ago and just received my results... I have a loss of disc height bulging disc with central annular disc protrusion causing minimal anterior effacement of the thecal sac...as well as right sided inferior neural foraminal encroachment from disc bulge. Also degenerative endplate marrow changes on the anterior inferior endplate. And last incidental Tarlov cysts. This is all within my L4 and L5, What do I do now?
