Diffuse disc bulge and tear

Patient : My MRI showed a mild diffuse disc bulge with small focal annular tear in left paracentral region. There is only mild narrowing of left lateral recess. Also a mild to moderate let foraminal narrowing at this level and mild right foraminal narrowing at L4 and L5. I experience left buttocks pain that radiates across my lower back and down my left leg incluing my knee area. At times there is tingling involved. It is worse after walking or sitting for long time periods. What is the best treatmet for this condition?
