Hamstring pain not related to an injury

Patient : My friend is a 49 yr old male that has suffered from muscle pain in the backs of his legs for as long as he can remember. he says that it is generaly in his hamstring and back of knee area. He describes the pain like twisting a rubber band operated propeller to its max. Usually a hot hot bath helps but for a short time. When he was a child he saw a Dr and they could not diagnose anything. Can you help?
