Having some issues with my 2 year old and trying

Patient : Having some issues with my 2 year old and trying to figure out if I need to have him seen at the UR/ER. He slept all day, woke up with a fever of roughly 101 from his nap, vomited liquid after denying solids and liquids most of the day. Very sluggish and lazy until I gave him a larger then normal dose of acetaminophen at 730pm. Until then he basically wouldn't talk and just laid there staring off I to space. He perked up for awhile after the meds kicked it. Took his temp at 11pm and it spiked to over 100 again. Gave him another dose even though it wasn't time yet. Breathing seems a bit labored but he has no cough, drainage, runny nose, symptoms of ear pain etc. Not sure what I should do for him.
