Patient : MRI of the lumbosacral region was done on 1.5 Tesla using a dedicated phased-array coil. T1 and T2 -weighted sagittal and axial imaging was done. Axial imaging was done through intervertebral discs. (CD containing MRI images given along with the films). Variable disc desiccation changes are seen in lumbar spine. Diffuse disc bulge with mild bilateral facetal arthropathy is seen at L3-4 level causing indentation on thecal sac. Diffuse disc bulge with mild bilateral facetal arthropathy and ligamentum flavum hypertrophy is seen at L4-5 level causing mild thecal sac compression and bilateral slight neural foraminal narrowing. Diffuse disc bulge with small sized posterocentral disc protrusion and mild bilateral facetal arthropathy is seen at L5-S1 level causing indentation on thecal sac and bilateral S1 nerve roots. Vertebrae show degenerative spondylotic changes with predominance of fatty marrow, suggesting changes of osteopenia. Mild marrow edema is seen along anteroinferior aspect of L4 vertebra, likely degenerative. No vertebral collapse is seen. CSF shows normal signal intensity. Lower end of the cord is normal. IMPRESSION - Diffuse disc bulge with mild bilateral facetal arthropathy and ligamentum flavum hypertrophy at L4-5 level causing mild thecal sac compression and bilateral slight neural foraminal narrowing. - Diffuse disc bulge with small sized posterocentral disc protrusion and mild bilateral facetal arthropathy at L5-S1 level causing indentation on thecal sac and bilateral S1 nerve roots.
