How do I get a prescription for physical therapy?

Patient : I have back pain and loss of strength in my left leg due to herniated discs. I am currently improving through physical therapy, but I need a doctor's order or prescription for physical therapy in order to get reimbursement from my health insurance. The doctor who ordered the MRI refuses to give me a prescription for PT because I did not choose surgery or get the therapy done at his hospital. Surgery was the only option he offered and I wanted to pursue conservative options first. What should I do to get the insurance coverage?
