It is not absolutely necessary to fast before an iron infusion, however some patient's develop nausea or vomiting secondary to the infusion, and rarely can develop an allergic reaction or anaphylaxis, therefore this is usually administered only in a hospital environment. It may be best to check with your local department, regarding their current infusion protocol.
