Is it a cold sore?

Patient : I've had a red bump on my lip for 4 days. It had a few small zit-like spots underneath that went away in 24 hours. I was worried it was a cold sore (I have no history of ever getting them before) so I started using abreva after about 36 hours. The bump doesn't hurt or itch, it isn't getting any worse (no ulceration) or better. Is there a way to know if it is a cold sore without seeing my doctor or waiting for it to ulcerate? I've been petrified to kiss or have any oral contact with my boyfriend, if it is a cold sore, how long should I wait before kissing or having sex with him? I have been diagnosed with perioral dermatitis that is flaring up, could it have anything to do with that skin conditon?
