Is this heart problems?

Q: Sometimes when I'm driving, my chest will tighten up and I feel a tingling in my upper body. This has happened a couple of times and I wonder if it has anything to do with caffeine. It usually happens when I stop for coffee early in the morning and haven't had anything to eat yet. It happens 1 to 3 hours after I've had the coffee and I feel like I am going to pass out. It's frightening because I'm usually driving on the freeway. Should I be concerned? I also have high blood pressure but I take medication and it's under control.
