Lethargic fourteen month old

Q: Hi. My 14 month old son has been extremely lethargic the past few days. He just got over a cold but his nose is not running anymore. He had a mild fever for 1 night and day we treated with infant tylenol. Last night he slept for 15 hours until I finally woke him up. After 1 hour awake, he fell asleep again so I let him sleep for 30mins. Then I encouraged him to eat and play for 2.5 hours although he was very lethargic the whole time. Then he slept for 3 hours again. Very lethargic, sluggish the rest of the evening and back to bed. We have noticed a lump on the back of his head on the right side near the base of his skull. One of his eyes is red. When he is awake and playing, he seems to fall over more than usual. Keeps losing his balance. He's never been like this in the past. Should we be concerned?
