Long Term Sleep Deprivation

Patient : What are my options for long term sleep deprivation. Medication or tested for sleep apnea, or possibly a blood test? I have tried to self medicate with a change in diet/exercise. I have lost over 30lbs in 7 months and continue to have very good nutrition and exercise. I have had a hard time getting to sleep and staying asleep for as long as I can remember. Its too much to bare now, I just want a good quality of life and not be tired all the time. I take no medications. I stopped taking birthcontrol because I thought it was making me tired. I intake enough calories, and drink enough water. I take multi-vitamins. I also stopped drinking caffeine 7 months ago. I am very healthy, I have a serious problem. I am asking for help now because I have exhausted all natural forms.
Symptoms:  Loss of short term memory, exhaustion, troubles remembering words, troubles thinking clearly, head pains, sadness. These symtoms go through waves of moderate to severe.
