Lower left back pain radiating to lower left abdomen

Patient : I am asking on behalf of a friend. 44 yr old male. He is having back pain in the lower left region that comes on suddenly then moves to his lower left abdomen. He says it is a deep, painful ache. The area on his back is slightly hot. The episode came on in about 20 minutes and now he is unable to sit or stand comfortably. Do you know what might be causing this pain?
