Military retiree in pain and I dont trust the VA

Q: Sir or Ma,am, I am a 45 year old military retiree after 21 years active duty. about five years before I retired I had to have knee surgery on my right knee and another one a year later. In 2005 my left ankle started to swell and get so painful I could barely walk. so after a lot of tests it was decided to get my left ankle fused and nerve damage was caused to the point it is hard to keep my balance. I also hurt severly hurt my back in 2000 and was place on bed rest for 10 days. Now the back pain is coming back again but even worse I can barely stand for more that 5 minutes before I have to sit down because the pain is great. All the VA will do is proscibe me NSAIDS, and about 4 to 5 months later the pain is back again. What are my options
