My 5 month old daughter has had a fever from

Patient : My 5 month old daughter has had a fever from about 10 a.m to now (2:30 a.m). It fluctuates between about 100.4 to 102 degree (oral thermometer- can't get a rectal reading). Her back and stomach are warm to the touch but the rest of her body is cool. She has been drinking her bottle throughout the day, but the amount has decreased from normal. She cries and gets very fussy every time she is moved, but eventually she settles down again and goes to sleep. When awake she moans a bit like she is absolutely miserable. She pooped 3x today- first it was green then it became yellow - normally it's 1-2x a day. I've been giving her Tylenol every 5 hrs since about 11. She also took a lukewarm bath. You can see the outline of her teeth through her gums and she has been biting recently so I wondered if she's teething. Should I go to the e.r?
