My father had a tracheostomy tube inserted through his stoma

Q: My father had a tracheostomy tube inserted through his stoma and when the inner cannula is in place he starts to cough really badly. When we remove the innet cannula he stops coughing, when the inner cannula is not fully pushed into the outer tube he does not cough. Why is this so? What should we do?
