Read and understood your history of pain in the shoulder on lifting movement.
The most important causes are:
Frozen shoulder, in which the tissues around the shoulder joint are inflamed hence gives the pain.
Trauma: due to fall or hit on the joint.
Diabetes must be ruled out.

I would advise you the following :
First of all consult an Orthopaedic Surgeon for clinical evaluation, physical examinations and investigations
MRI is the best to tell the extent of the disease and guides for further management.
