Need to know if I am pre-diabetic and if there is a thyroid disorder
Patient :Is a reading of 10.2 mIU/1 high when tested on insulin fasting
Symptoms: I am a female, 30 years of age. My initial weight before giving birth, 8 years ago, was 59kg. Perhaps two years after I began putting on weight, until I reached 80kg. My doctor suspected Polycystic ovary syndrome, and scans did pick up something to confirm. I was then put on medication. Later, two scans were done, and no signs of any cysts were found.
To loose weight, I tried a TLC diet (www.tlcforwellbeing.com) and lost twenty. Then two years ago, slowly my weight gain started again, and now the 20 kg has returned.
I visited an endocrinologist, who sent me for the following tests:
Glucose fasting : Result 5.0
Ferritin Result 75
TSH Result 1.93
Free T4 Result 12.3
Insulin fasting Result 9.0
Antithyroglobulin Result 16
Antithyroid Peroxidase Result 1
Cortisol Result 188
Total Testos 2.4
Free testosterone calculated 44.7
OH vitamin D 4.8 (Note made L- severe deficiency) It must be remembered that I am in the veil thus I do get very little exposure to sunlight.
Androstenedione 16.2 (Note made H (high) - insufficient sample to repeat the test on an alternative method to exclude a possible interfering substance as cause of the increased result)
S-LDL Cholesterol 3.2 (Note made H)
S-HDL Cholesterol 1.3
S-Non HDL Cholesterol 3.5
S-CHol/HDL Ratio 3.7
Besides for three places, i.e. Vitamin D (4.8), Androstenedione (16.2) and S-LDL Cholesterol (3.2) the blood results made no other note. My doctor however, besides these 3 issues, felt that my insulin fasting was too high (9.0) and that I had an under-active thyroid issue.
He put me on : euthyrox 25 ug (half tablet every morning)
glucophage (2 of 500mg every evening
Calciferol 1 tablet three times a week
After 6 months I was told to take blood tests again for:
LDL Cholesterol result : 2.9 (lower than the 3.2 six months ago)
Glucose fasting result : 5.5
Insulin fasting result : 10.2 (6 months ago it was 9.0)
Homa index result : 2.49 (Note made that H (high)
Free T4 13.3
Free T3 4.0
(Note made : Thyroid function biochemically normal)
Cortisol random 372
The results did not comment on any thyroid or insulin problem, but my doctor felt that my fasting insulin is too high and now increased by medication to:
euthyrox 25 ug (2 tablets every morning)
glucophage (2 of 500mg morning and at night)
vitamin D supplement (25000 units 1 three times a week)
Before starting the new dosage I showed my results to a homeopath who felt the my fasting insulin levels were not so high, nor was there a real sigh of an under-active thyroid, and advised me to stop the medication, except for the vitamin D supplement. The homeopath felt that I had hormonal problems, which has affected my weight, and nothing else.
