No period in over a year!

Patient : My wife is 27 years old and we have been trying to have a baby for over a year not but she will not conceive. She has not had a period in over a year and all pregnancy tests have came back negative. She shows the signs of being pregnant but is clearly not. She has been gaining weight and having sharp pains in the area of the stomach. The last period she had was about 13 months and was VERY heavy bleeding. I am not sure whats going on and its worrying me. She shows the signs of having PCOS. Is there anyway we can find out for sure?
