Perineum tear

Patient : I am a 27 year old 10 weeks postpartum. I had a 4th degree tear during delivery. At my 6 week check up everything to seemed fine. I had an IUD inserted and started spotting about two weeks ago and then I noticed blood on the back of my pad and got to looking and realized I had a tear close to my rectum. I went to the doctor Thursday and he stitched it up. I was in so much pain afterwards that I didn't really think to ask any questions. 1) when showering should I use soap? 2) how often should I clean the area? 3) I was prescribed estrogen cream and was instructed to put in the vaginal region all the way back over stitches and the rectum area. What is this for? 4)I am having some fecal incontinence is this normal?
