Queried Left PACI

Patient : Hi, I am currently working (as a speech therapy student) with a lady in a stroke unit. I am attempting to determine her neurological status. From a brief look at her medical chart I have noted the following; 1) MRI revealed chronic ischemic changes. No evidence of hemorrhagic. 2)CT revealed a small area of infarction The medical team queried a left PACI. My question is why is it queried and not a diagnosis? Also, if the CT revealed a small area of infarction (should this area be identified on CT report)? Thanks You
