Severe nerve twich when falling asleep

Patient: When I am just about to fall asleep I have a severe nerve jerk or twich that at this point wakes me and anyone in the same room up. It started last year with just some leg twiches but has been getting worse all the time. Right now I swear I jump off the bed and make a loud grunt. This can happen 20 or 30 times before I fall asleep. My abs even hurt from the violent contractions.
