Sharp pain in lower back

Patient : At L-4 L-5 I have an artificial disc done 10 years ago. Until 8/14/13 I had no problems. I hurt my back again pulling on a fifth wheel release handle. Sharp pain in the lower back and unable to sit, stand or walk for more than 20 minutes. I did 1.5 months of physical therapy with no results. Had a MRI it showed L4-L5 facet hypertrophy with narrowing of both neural foramina. As well as moderate spinal stenosis. L5-S1 showed again hypertrophy and narrowing of the inferior aspect of both neural foramina. My question is what do I need to get better my Dr told me we are born that way but articles I've read say something different. Any advice would be greatly appreciated.
Symptoms:  Low back, both hips, both but cheeks, right groin and occasionally pain down entire right leg.
