Should I go see a doctor in person?

Patient : I haven't doing so great in school. As a child i was good until I entered high school and then things changed slowly over time. Whenever I meet someone or talk to someone, I seem to change topics constantly. My first two years of college I did terribly bad even when I studied because I would end up being distracted or just not have things organized for me to study or I would be clueless. So my juniot year of college, which is now, I started to smoke weed. I smoke weed about two times a day to help me concentrate in class. My scores have been increasing but I still hate smoking weed; i hate the smell of it, and I don't even want it on my clothes. I am scared to go to a doctor and tell them about this situation... Any ideas on what I should do?
