Shoulder issues

Patient : 34 YR. OLD MALE injured shoulder at work. Shoulder pops in and out of place. Hand and fingers are numb 24 hrs. a day. Shooting pain down my arm. M.R.I. completed. Impression: Glenohumeral osteoarthric changes with fissuring of the glenoid labrum. Acromioclavicular osteoarthritis. I have been to 10 therapy sessions with no success. Arm and shoulder are getting worse. Nerve induction test was also a failure. Is the next step Surgery? They cant expect me to live with this pain for ever, can they?
