Tilted knee cap problems

Patient : Hi i am a 20 year partially dislocated my knee cap last year had two mri scans which showed i have a tilted knee cap. Have been at physio over 6 months and still have alot of pain. I am currently waiting on getting surgery to correct the tilt .The consualnt spoke about cutting away the tendon so it sits back in place but didnt explain much apart from it will be done as key hole but my second scan showed swelling of fat pad but he says the tilt is causing me the pain should i be going ahead with such surgery so young would the fat pad swell cause of tilt?
