Upper Back and Neck Pain

Patient : I have had a headache for over two years now (without a break) and had an MRI that was normal. about 6 months ago the upper back pain started. It has been getting worse and there are more tender areas. The pain is severe at night but it's even worse in the morning when i wake up. Laying down is painful but after sitting or standing for a while, my back needs to rest. There are extremely sensitve spots all over my back but the worst ones are in my neck, in my shoulders, and in my right rib cage. I am not as worried about the headaches, I have been informed that they will not go away for a long time, but the back pain is extremely painful and I am not sure what could be causing it. Does anyone have any idea as to what is causing this?
