UTI, yeast infection or chlamydia?

Patient : I've had an issue with my penis for almost three months. Red around opening of urethra, very dark and foul smelling urine, penis head looks shriveled with striations throughout, occasional sharp pain in left side of body, no discharge of any kind from penis, typically no burning during urination however towards the end of using the bathroom I get a very weak flow to the point where urine dribbles out, most recently in the last two days my testicles have become sore. Is this a yeast infection, urinary tract infection, or possibly chlamydia?
