What are the chances of my ex-girlfriend getting pregnant?

Q: I broke up with my ex girlfriend 3 months ago, and we have not had intercourse for this period of time. She has PCOS (polycystic ovary syndrome) which she was using the contraceptive pill Levast for cycle management, and has had regular periods ever since; every time we did have sex I was sure to use a condom and there were never any problems with the condom tearing/ripping as to my knowledge. So what is the possibility of her being pregnant given that we were using birth control and the pill combined with us not having sex for 3 months?
